Informed consent is a conversation and decision process, not simply a signature. Your clinical team should explain the expected benefit, important risks, reasonable alternatives and what may happen if the procedure is not performed.
Before Signing a Form
- Confirm the exact procedure and why it is being recommended.
- Ask about common side effects, important uncommon risks and alternatives.
- Tell the team about medicines, allergies, pregnancy, kidney disease and previous reactions.
- Ask for an interpreter if you cannot fully understand the language used in the form.
Official Documents
- The Turkish Society of Cardiology publishes official Turkish consent forms for angiography, stenting, ablation, pacemakers, ICD, CRT, TAVI and other procedures.
- The current form used by the hospital or clinic where the procedure is performed is the document that must be reviewed and signed.
