Leg Pain When Walking

Peripheral artery disease, musculoskeletal and nerve causes of walking-related leg pain; ABI testing and urgent limb symptoms.

Calf, thigh or buttock discomfort that appears at a similar walking distance and improves with rest may be intermittent claudication from peripheral artery disease. Spine, joint and muscle conditions can mimic it.

Why the Symptom Pattern Matters

  • Reproducible cramping with walking and relief within minutes of rest suggests arterial claudication.
  • Relief mainly by sitting or bending may suggest spinal stenosis.
  • Rest foot pain, wounds or colour change can indicate advanced ischaemia.
  • A sudden cold painful limb is an emergency.

  • Neurological and joint examination

Which Tests May Be Considered?

Not every test is required for every person. Selection depends on examination findings and clinical probability.

  • Ankle-brachial index
  • Exercise ABI when resting results are non-diagnostic
  • Vascular ultrasound
  • CT or MR angiography for procedural planning

What to Record Before the Appointment

  • Distance to pain onset
  • Time to relief
  • Foot wounds or colour change
  • Smoking and walking capacity

When to Call Emergency Services

Do not wait for an online reply
  • Sudden severe pain with a cold pale or blue limb
  • New numbness, weakness or inability to move the foot
  • Rapidly blackening wound or spreading infection

Frequently Asked Questions

Is every calf cramp a blocked artery?

No. Pattern, pulse examination and ABI help distinguish causes.

Is walking harmful in PAD?

When acute limb threat is absent, structured walking is an important treatment.

Do palpable pulses exclude PAD?

No. Examination is useful but does not exclude milder disease.
