Leg Swelling

One- or two-sided leg swelling, heart failure, venous thrombosis, medicine effects and emergency warning signs.

Leg swelling can result from fluid retention, venous or lymphatic disease, a clot, infection, medicine effects or organ disease. One-sided sudden swelling and gradual swelling of both legs have different risk patterns.

Why the Symptom Pattern Matters

  • Both legs with weight gain and breathlessness may suggest congestion.
  • Sudden one-sided pain and swelling raises concern for deep vein thrombosis.
  • Swelling that worsens through the day may occur with venous disease.
  • Redness, heat and fever may suggest infection.

Which Tests May Be Considered?

Not every test is required for every person. Selection depends on examination findings and clinical probability.

  • Venous ultrasound when DVT is possible
  • Kidney, liver, thyroid, albumin and urine tests
  • ECG, natriuretic peptide and echocardiography when heart failure is suspected
  • Imaging selected from associated findings

What to Record Before the Appointment

  • Morning weight
  • Side and daily change
  • Skin colour, wounds and calf pain
  • Breathlessness and pillow requirement

When to Call Emergency Services

Do not wait for an online reply
  • Leg swelling with sudden breathlessness, chest pain or coughing blood
  • Severe rest breathlessness or pink frothy sputum
  • A suddenly cold, pale, blue or very painful limb
  • Pregnancy with severe headache or visual symptoms

Is all leg swelling heart failure?

No. Venous, medicine, kidney, liver and lymphatic causes are common.

Should I take a diuretic?

Not without identifying the cause and having a safe monitoring plan.

Why is one-sided swelling important?

It may indicate DVT, which can lead to pulmonary embolism.
