Ventricular Tachycardia and PVC Ablation
How ventricular arrhythmia ablation is planned, performed and followed in patients with VT or frequent PVCs.
Catheter ablation targets the focus or electrical circuit responsible for ventricular tachycardia or frequent premature ventricular contractions. The goals may be symptom relief, reduction of ICD shocks, control of recurrent VT or recovery from PVC-induced cardiomyopathy.
Planning
Evaluation includes ECG morphology, rhythm monitoring, echocardiography and often cardiac MRI to define scar and ventricular function. Coronary disease, inflammatory disease and inherited cardiomyopathy are considered. The expected benefit and procedural risk differ substantially between an isolated PVC focus in a normal heart and scar-related VT in structural heart disease.
Procedure and Follow-up
Three-dimensional mapping identifies the origin or circuit. Ablation may be performed from inside the heart and, in selected cases, from the epicardial surface. Success is assessed with arrhythmia inducibility, rhythm burden, symptoms and ventricular function. Medicines or ICD therapy may still be needed because ablation does not remove all future arrhythmic risk.
Fainting, sustained rapid rhythm, ICD shocks or severe chest pain require urgent medical assessment.
Patient Selection and Procedural Planning
The procedure is considered only when the expected diagnostic or therapeutic benefit is meaningful for the individual patient. Anatomy, prior imaging, symptoms, rhythm or haemodynamic findings, kidney function, bleeding and vascular risk, medicines and reasonable alternatives are reviewed before consent.
Technical Goals and Limitations
Procedural success requires a defined endpoint, but an immediate technical result does not guarantee permanent symptom relief or eliminate the underlying cardiovascular disease. Complex anatomy, previous procedures and associated disease can change technique, success probability and complication risk.
Risks and Safety
Potential risks vary by procedure and may include bleeding or vascular injury, cardiac perforation, rhythm or conduction disturbance, contrast or kidney effects, stroke, infection and the need for urgent additional treatment. The treating team explains the risks that are specifically relevant to the planned approach.
Recovery and Follow-up
Written instructions should cover access-site or wound care, medicines, driving, work, physical activity and warning signs. Follow-up confirms the procedural endpoint, symptom response and any required imaging or rhythm monitoring. Never stop antithrombotic or rhythm medicine without a procedure-specific plan.
